On November 10, 2004, during Operation Phantom Fury in Fallujah, Iraq, Bellavia's squad was tasked with clearing a building. Despite being outnumbered and outgunned, Bellavia led his men in a fierce firefight, neutralizing multiple enemy positions and rescuing a wounded comrade.

For his extraordinary courage and selfless actions, Bellavia was awarded the Medal of Honor, the United States' highest military honor.
